What to do in the event of a claim

Action Required:

Property

  1. Do whatever is necessary to prevent further damage from occurring.
  2. Report all incidents - failure to report an incident that gives rise to a claim could prejudice both you and/or your insurers rights of recovery.
  3. Call Or e-mail Affinity (see incident report form - PDF, 15kb) to request a claim form and advise the following information to ascertain if a Loss Adjuster needs to be appointed:
    • Date Damage Occurred:
    • What Happened.
    • Advise Estimate for repair/replacement.

Liability

  1. If person is injured, call for medical help.
  2. Report all incidents - failure to report an incident that gives rise to a claim could prejudice both you and/or your insurers rights of recovery.
  3. Contact Affinity Insurance Brokers (See incident report form) to report the incident?
  4. Gather statements from all witnesses present?
  5. Take photos of the accident location?
  6. Bear in mind for example, in the case of a faulty piece of equipment or fault with design of location that a third party (or any person who supplies a product or service) could indeed be brought into the equation, contact details should be forwarded to your broker with the accident report form.
